Thread lifting is a procedure that pulls descended tissue and repositions it, while fat grafting is a surgery that fills hollow areas with your own fat. The approach depends on whether the problem is tissue that has descended or areas that have become hollow, and when both are present, the two are sometimes combined.

Fat grafting differs in that it uses your own tissue, so there is less concern about foreign-body reactions. However, the graft survival rate varies from person to person and surgery and recovery are required, so we discuss the choice considering the area, the amount of volume you want, and your recovery circumstances.

The timing is decided after checking when the grafted fat will settle and the current condition of the tissue. There have been cases in which patients who came for a consultation less than 3 months after fat grafting were treated safely after their facial condition and recovery were thoroughly checked.

Fat is grafted to areas lacking volume, such as the forehead, eyelids, front cheeks, nasolabial folds, under the eyes, temples, and chin. The amount needed differs by area, so the graft volume is determined according to the degree of hollowness.

The surgery takes about 1 hour and is performed under sedation. The time may vary depending on the number of harvesting and grafting sites.

Recovery takes about 7 days as a guideline, and sutures are removed around day 10. Swelling and bruising are most noticeable on days 1–3 and then gradually subside, but the pace varies from person to person.

We decide by looking at the recovery of both the harvest site and the graft site. We check your condition at suture removal and follow-up visits and advise you on the timing individually.

Sagging and hollowing have different causes. If tissue moving downward is the main cause, lifting is needed; if volume loss is the main cause, pulling alone may not achieve the change you want. We evaluate both before deciding on a direction.
