However, through incision and removal of excess skin and fat, we can create sharper, more defined lines. Precise suitability is determined through consultation and diagnosis.

Burial and natural adhesion methods create lines without incisions and are suitable for thin-skinned eyes with minimal laxity, offering faster recovery. Conversely, incisional double eyelids directly remove skin and fat, making them suitable for thick or sagging eyelids with longer-lasting results. Since the incision follows the double eyelid line, it is often hidden within the fold after healing and becomes less noticeable. However, scar appearance and healing vary depending on individual skin type and constitution. We monitor progress through aftercare.

Sutures are typically removed around 7 days post-surgery, after which swelling gradually subsides and the line settles. Incisional double eyelids have a somewhat longer recovery than burial or natural adhesion methods, and complete swelling resolution may take longer depending on the individual.

The difference is the length of the incision. Partial incision opens only part of the line to remove fat and fix the line, so the incision is smaller, while full incision opens the entire line to remove loose skin and fat more extensively. The required extent depends on upper eyelid thickness and the degree of skin sagging, and we explain which extent is appropriate after diagnosis.

If the buried-suture crease came undone because of thick upper eyelids or sagging skin, refining the tissue with an incisional method may help the crease last better than redoing it the same way. However, the cause of the crease coming undone differs from person to person, so the method is decided after checking when the previous surgery was done and the current condition of your eyes.

We remove only as much as needed. Removing too much fat can make the upper eyelids look hollow and actually make you look older, so we decide both how much to keep and how much to remove. We design the extent of excision after assessing fat volume, skin thickness, and the degree of hollowing.

If sagging skin is enough to block your vision, creating a double eyelid line alone may not be enough, and we consider upper blepharoplasty to remove the loose skin or eye shape correction (ptosis correction) as well. The direction depends on how much sagging there is and whether your eye-opening strength is weak, so we decide after diagnosis.

Incisional double eyelid surgery takes about 40 minutes to 1 hour and does not require hospitalization. It is performed under local anesthesia or sedation, followed by 1–2 visits to check your progress.

On the day of surgery, it is best to keep applying cold compresses and to avoid rubbing your eyes or bending forward.
